All-on-4 dental implants are one of the most advanced and convenient solutions for patients who have lost multiple or all of their teeth. The concept is straightforward: just four implants support a fully fixed set of teeth – placed in a single treatment day. Many people postpone dental restoration for years due to fear of pain, lengthy procedures, or high costs.
All-on-4 is primarily recommended for patients who have lost multiple teeth, or whose remaining teeth can no longer be saved. It is an excellent alternative to traditional removable dentures, offering a fixed, stable, and natural feel. The method is particularly beneficial in cases where bone volume is insufficient for conventional implants, as All-on-4 implants are placed at an angle to make the most of available bone. Annual dental check-ups can help prevent total tooth loss – but if it has already occurred, All-on-4 is one of the best paths back to natural chewing function and aesthetics.
The first step is a detailed dental consultation and X-ray examination (usually a 3D CBCT scan), during which the dentist assesses the quantity and quality of the bone and plans the positioning of the implants. Typically in a single day, any remaining teeth are extracted (if necessary), the implants are placed, and a temporary set of teeth is fixed in position. This is the principle of “immediate loading” – meaning the patient leaves the clinic the same day with a functional, aesthetic set of teeth. The final porcelain prosthesis is usually fitted 3–6 months later, once the implants have fully integrated with the bone.
The temporary prosthesis is typically made from acrylic, which is a lightweight and cost-effective solution for the healing period. The final prosthesis is most commonly made from porcelain (zirconium dioxide), which is extremely durable, aesthetically pleasing, and biocompatible – meaning it does not cause allergic reactions. The size, shape, and shade of the teeth can be individually designed to achieve a natural appearance. When selecting materials, the dentist takes into account the patient’s preferences, chewing load, and long-term durability requirements.
The procedure is carried out under local anaesthesia, so the treatment itself is painless. Sedation (conscious sedation) is also available upon request, further increasing patient comfort. In the days following surgery, mild swelling and tenderness may be experienced, which can be effectively managed with pain relief medication.
Most patients return to their daily routines within 3–5 days. Softer foods are recommended during the first few weeks to allow the implants to integrate undisturbed. If any unusual symptoms occur – such as severe pain, fever, or persistent swelling – contact your dentist immediately.
With proper care, All-on-4 implants can last for several decades. The final porcelain prosthesis typically functions flawlessly for 10–15 years or longer. The key to longevity is regular dental check-ups (at least once a year), proper oral hygiene (use of interdental brushes, dental floss, and mouthwash), and protection against teeth grinding. It is worth noting that the implant fixtures in the bone generally last a lifetime, while the prosthetic teeth may eventually need replacing.
The cost of All-on-4 treatment depends on several factors: the brand and type of implants used, the material of the prosthesis, any preparatory procedures required (e.g. extractions, bone grafting), and whether one or both jaws are being treated. The final porcelain prosthesis is typically ready 3–6 months after the implants are placed. This time is needed to allow the implants to fully integrate with the jawbone (osseointegration). During the healing period, the patient wears the temporary prosthesis, which allows for normal eating and speaking. Once the final prosthesis is fitted, it is precisely adjusted to the individual's bite and aesthetic preferences.
No – the All-on-4 prosthesis is fixed, meaning the patient cannot and does not need to remove it. This is one of its greatest advantages over traditional removable dentures: no dental adhesive is needed, it does not shift, and it causes no discomfort while eating or speaking. The aesthetic and functional result is very close to that of natural teeth. Cleaning is straightforward with interdental brushes and mouthwash at home.
One of the main advantages of All-on-4 is that bone grafting is usually not necessary, unlike with conventional implants. The angled placement of the implants makes it possible to maximise the use of available bone. In some cases – such as severe bone resorption – preparatory procedures may still be needed. Your dentist will provide precise information based on the results of a 3D X-ray scan.
All-on-4 uses four implants, while All-on-6 uses six to support the prosthesis. Six implants provide greater stability and load-bearing capacity, which is why the dentist may recommend All-on-6 in certain cases – particularly for the lower jaw or where greater chewing demands are expected. However, All-on-4 is entirely sufficient for the majority of patients and comes at a more affordable price. The decision is always based on bone volume, age, general health, and individual needs.
All-on-4 may be contraindicated for patients who are active smokers (smoking significantly reduces the chances of successful osseointegration), have uncontrolled diabetes, suffer from blood clotting disorders, or have previously undergone radiation therapy to the jaw area. Certain medications (e.g. bisphosphonates) may also affect implant success. During the initial consultation, the dentist will carry out a thorough assessment to determine whether All-on-4 is a safe and appropriate option for each individual patient.
